The program price includes the study materials and the examination fee. Registration is required 14 days before the program to allow us to get you the study materials and for you to have time to read and study the materials.

A $50 nonrefundable deposit is required to hold the registered spot, and will be retained as a cancelation/rescheduling fee if the requested class is not attended. Please complete and retain the emailed copy of this form. Payment is required at the time of registration and prior to our sending you study materials unless previous arrangements have been made, such as by using an approved purchase order. For organizations who post-pay via invoice, the same $50 fee will be due and payable for cancellation or rescheduling.
